The American Fork Family Clinic of Wasatch Mental Health provides mental health services for adults, children, and families. We offer a variety of therapeutic services designed to help individuals and families who are affected in some way by mental illness.
Eligibility for Services:
You must have Medicaid to receive services at Wasatch Mental Health. If you have Traditional Medicaid (pink card), your services will be covered at 100%. If you have Non-Traditional Medicaid (blue card), your services will be covered at 100% but you are limited 30 visits per calendar year. Wasatch Mental Health does not accept PCN Medicaid (yellow card), CHIP, or any other private insurance.
How to Become a Client:
Getting started at Wasatch is a friendly and simple process. At the American Fork Family Clinic we have walk-in times each week for new clients. The intake information is as follows:
• The walk-in intakes are each Monday and Thursday at 9am
• The intakes are on a first come first serve basis so please arrive on time
• Children must be accompanied by their legal guardian
• The client must bring their current Medicaid card
• The intake will take about 2 hours
