If you are a new client (regardless of age), the process begins here. This is where all evaluations take place before a client is allowed to use the services provided at Wasatch Mental Health. An intake therapist will meet with you to collect initial information. An evaluation of your symptoms will take place so you can be appropriately referred to a doctor, a therapist or any other needed service. The initial evaluation takes about 1 1/2 hours. If you need emergency assistance after hours, call (801) 373-7393. Preparing to Meet Your Psychiatrist This form is used to better prepare to meet with your psychiatrist. The form will help you organize your medication observations and any concerns you might have about medication actions and reactions.
