The School Based Services program provides onsite programs for children in special behavioral or emotional programs in their schools. The program covers three school districts: Alpine, Provo and Nebo. The SBS (School Based Services) Program has eight programs in 28 schools in Alpine, Nebo and Provo School Districts. Site Based provides services for
children in special emotional small group classrooms in their schools.
The child must be referred through the school district for this
program. The services offered include individual therapy, group
therapy, case management, social skills groups and coordination with
parents. Elementary teams consist of a therapist and case manager.
They deliver services upon referral from the school coordinators. All
school based services are provided in the schools. Alpine School District At East Shore High School we have a therapist and a case manager. Alpine School District has small group classrooms in Aspen, Lindon and Rocky Mountain Elementary Schools, Canyon View, Pleasant Grove and Willow Creek Junior High Schools and Lone Peak High School. Charity Tanner, 812-5261, is the therapist and Staci Oates, 812-5263, is the Case Manager. Site Based Services in Alpine School District are coordinated by Matt Madsen-227-2440 Alpine Elementary services are in six schools: Westmore, Geneva, Sharon, Pony Express, Eagle Valley and Greenwood Elementary Schools. Kristen Whittenburg 812-5253 is the therapist and Tammy Baker- 812-5271 is the case manager. Provo School District Provo Work Program is coordinated through Ron Whitney 812-5275. Provo Onsite Program is coordinated through Clyda Lindsay 812-5270. She works in Canyon Crest Elementary, Dixon Middle School and Provo and Timpview high Schools. Provo Elementary serves four schools: Franklin, Farrer, Springcreek and Timpanogos Elementary schools. Elizabeth Feil- 812-5297 is the therapist and Melissa Charles, 812-5275, is the case manager. Services can be coordinated through either of them. Nebo School District Nebo Elementary serves 6 elementary schools - Brockbank, Goshen, Larsen, Westside, Santaquin, and Wilson.
