Supported Housing is one of the programs at the South Provo Clinic. Eligibility for supported housing requires the following:
- Utah County resident
- Medicaid
- Completion of a Mental Health Assessment to determine whether your needs meet
this level of care.
The following supported housing options are available through Wasatch Behavioral Health:
Alpine House– 156 South 300 West, Provo, Utah
This is a United Way agency providing supportive residential services from live-in house parents. Supervision is provided throughout the week, including holidays. Individuals must have activities away from Alpine House daily between 9:00 AM to 3:00 PM and be involved in treatment through WMH.
Supported Residential Treatment (SRT)– 911 South 950 West, Payson, Utah
This facility provides independent living apartments with supervision from on-site house parents. Residents have the opportunity to practice skills of daily living, self care, symptom management, and other structured care for stable community life. Residents must be involved in treatment through WBH.
Payson Independent– 911 South 950 West, Payson, Utah
These are independent living apartments with minimal supervision from on-site house parents. Residents must be involved in treatment through WBH and sign a one year lease.
Mapleview– 1193 East 300 North, Provo, Utah
This is a complex of independent living apartments managed by Provo City Housing. Residents must be involved in treatment through WBH and sign a one year lease.
Yarrow Apartments– 210 East 300 South, Springville, Utah
This is a complex of independent living apartments owned by the Housing Authority of Utah County. Residents must be involved in treatment through WBH and sign a one year lease.
How to Become a Client:
If you are a current client of WBH talk to your therapist and case manager about your housing needs and they will refer you accordingly.
If you are not a current client of WBH complete an intake through the Westpark Family Clinic in Provo.
