Wasatch Behavioral Health employees, if you need your 1095-C (Medical Insurance Tax form), these can be found by simply logging into your account at employeenavigator.com . Once logged in, to the far right, under ‘Actions’, select, ‘view 1095 for 2025’. From there, you are able to save/print the PDF.
