How do I get a copy of my medical records from WBH?
Fill out an Authorization Form and either e-mail, fax, or return in person to any Wasatch Behavioral Health office.
What are the fees for obtaining copies of my medical records?
- Client (or parent/guardian) requests – No charge for first request during a calendar year, $5.00 for subsequent requests, then, no charge for electronic delivery or $.25 per page for physical copies. Per-page fee is paid at the time the records are picked up (initial deposit may be more depending upon amount of information requested).
- Attorney/Record Retrieval Companies – $21.00 research fee, then $.53 per page for the first 40 pages then $0.32 for each page after 40 pages. Expedite fee is $25.00, separate from other charges. *WBH payment agreement must be signed prior to processing records request.
- Other Treatment Providers – no charge
